Download an application

You need to fill in the application and mail or fax it to the school. The mailing address and fax number are in the masthead at the top of these pages.

To download the enrollment application, right click on the link, and then click on "Save Target..." to save the file: to open the enrollment application in your browser, click on the link.

In order to print this application, you will need Adobe Acrobat Reader. If you do not have Acrobat Reader installed, you may get if by clicking here.
